This
rider may only be availed if attached to a Group Yearly
Renewable Term Policy.
Sun Life Philippines shall pay to such Employee, if
living, or to his beneficiary the corresponding percentage
of the Accidental Death, Dismemberment and Disability
Insurance in force for such loss on such date:

Only one of the amounts shown above (the largest applicable)
is paid for injuries to the same limb resulting from
any one accident. Losses sustained as a result of any
subsequent accident shall be considered for payment
separately from and independently of other losses sustained
in a previous accident. The maximum amount of Accidental
Death, Dismemberment and Disability Benefit payable
for losses resulting from any one or more accidents
in any policy year is 100%.
Loss of an arm means severance at or above the elbow.
Loss of a leg means severance at or above the knee.
Loss of hand or foot means severance of hand or foot
at or above wrist or ankle. Loss of sight must be total
and irrecoverable. Loss of speech means total and irrecoverable
loss of speech. Loss of hearing in both ears means the
total and irrecoverable loss of hearing. Loss of thumb
and index finger on the same hand means severance through
or above the metacarpophalangeal joints.
Loss of use of a limb must be total, continuous for
12 whole months, and then must be determined to be permanent
and irrecoverable before the benefit is payable.
